National Student Exchange Forms
To apply for a basic exchange, you need to complete and return the first four forms. If you want to do a resident assistant exchange, add the fifth form to your application. If you want to go to any school in Puerto Rico, Université du Québec à Montréal, or Université de Sherbrooke, add the sixth form to your application.
